Promotion Terms

“Refer Your Friend for Chiropractic or Massage and Receive a FREE Massage” Promotion Details and Terms

  1. On every card you hand out, make sure to first write in print your full name and phone number on the back of the card.
  2. There is no limit to how many cards you can hand out or people you can refer.
  3. Come to our office or call the office to get more referral cards to hand out.
  4. The person you refer must come in to our office and present the referral card on their visit in order for you to get your free massage.
  5. After the person you referred has come in for their visit, Lifestyle Chiropractic office staff will call the number written on the back of the card only TWICE to schedule your free massage. If a message is left, it is your responsibility to call Lifestyle back to schedule your free massage.
  6. When the promotion period ends, LC office staff will give a prize to the person who referred the most number of people.
  7. Promotion end date is December 31, 2009. Promotion may be subject to extension.
  8. Promotion subject to change. Notice will be posted to this web site.
